Organized through Coach Murr, this combo includes a sleeveless shirt as pictured, decorated on the front with the Dragons logo, and decorated on the back with the players last name and number. 
 
Visor will be embroidered with "Dragons" similiar to the Dragons on the front of the shirt. 
 
Please mark in the fields below the players name and number that needs to be populated on the Jersey.  Please coordinate your player's number with Coach Murr so there are not two players with the same number before ordering. 
 
You can pay by card, and choose the "pick up" delivery option, and once all of the shirts are complete, we will deliver to Coach Murr so that he may hand them out at one time. 
 
If you prefer to have them delivered via USPS then we can do this but it will be an additional charge.   (we have a $3 flat fee for shipping)
